Arizona regulations state that individuals must successfully complete an approved CNA training in Arizona with at least 120 hours.Pass both written and skill CNA State Exam.  You must submit fingerprint cards for results of a state and federal background check in order to work in Arizona as a CNA.  Only persons who complete an approved RN or LPN education program can challenge the competency exam.

Arizona CNA Certification Renewal
CNA certificates are renewed every two years.  CNA's are required to provide proof of employment every 2 years such as

  • a copy of a pay stub if the EMPLOYER’S name and the CNA’s name are on the pay stub
  • a copy of your W-2 form
  • a completed Proof of Employment form

Transferring Your CNA Certification to Arizona

How To Reinstate Your CNA Certification In Arizona

Any nurse aide in Arizona whose certificate was lapsed, revoked or voluntarily withdrawn must make a written request to the Arizona State Board of Nursing before they can reinstate their certification. Additionally, they may be required to re-attend training, take the CNA competency exam again and pay a reinstatement fee to reinstate their certification.
